As the visitors became increasingly frantic in the second half, David Luiz rose highest to glance Hazard’s corner inside the far post.
Pep Guardiola’s side, beaten in the league for the first time since April, slip to second, a point behind Liverpool.
Chelsea are a further seven points back, jostling for third place with Arsenal and Tottenham.
